Consent to Use Image and Testimonial for Promotion

I give my consent for the Provider to use my photo, testimonial, or similar content for promotional purposes. I agree that any written testimonial I provide can be used by the Provider as they see fit.

Agree to give consent to use my photo, testimonial or similar for advertising purposes.

Health Acknowledgement *

I acknowledge and confirm that:

I do not suffer from, nor am I impaired by, any condition listed as a Restricted Condition by the Provider, including but not limited to: heart conditions, epilepsy, high or low blood pressure, recent surgeries, pregnancy, or other medical issues that may make participation unsafe or, if I do suffer from a Restricted Condition, I have obtained a Doctor’s consent to participate in my selected Modalities, which I will provide to the Provider; and

I have disclosed to the Provider any other relevant medical conditions, injuries, illnesses, or impairments which may affect my ability to safely participate in the recovery modalities offered (including Ice Baths, Magnesium Baths, and Compression Boots).

I understand that participation in the recovery services (Modalities) carries inherent risks, including but not limited to slips, falls, shock, allergic reaction, or other adverse health events. I agree to immediately notify the Provider’s staff if I feel unwell, faint, dizzy, uncomfortable or experience any symptoms that may indicate an adverse reaction. I agree to comply with all reasonable instructions or directions provided to me during participation.

Acceptance of Risk and Release *

To the maximum extent permitted by law, including the Australian Consumer Law, I:

freely assume all risks associated with my participation in the Services;

release and forever discharge Invigorate Recovery, its officers, contractors, staff, agents, and affiliates (the "Released Parties") from all Claims, including for personal injury, property damage, or loss of any kind arising directly or indirectly from my participation in the Services; and

acknowledge that this waiver operates as a bar to any Claim or proceeding I may otherwise bring against the Released Parties in connection with the provision of Services, except where liability cannot be excluded under law.

Legal Rights and Acknowledgement *

I acknowledge that, I am not relying on any representation or warranty from the Provider outside the written terms; and Nothing in this document limits any rights I may have under the Australian Consumer Law which cannot lawfully be excluded.
